Licensed Practical Nurse

About the role

Oakhurst Health and Rehabilitation is seeking compassionate and committed Licensed Practical Nurses (LPNs) to join their clinical team. This is a great opportunity for individuals who want to make a difference in the lives of seniors.

Responsibilities

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines
• Administer medications and treatments with precision and compassion
• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Absorb and assist with resident comfort
• Collect samples and monitor catheters. Provide wound care
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration
• Help coach staff as necessary

Requirements

• Current LPN license in Virginia
• CPR certification (or willingness to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment

Pay

$30.50 to $34.87 per hour (plus shift differentials)
