Licensed Practical Nurse
About the role
The Licensed Practical Nurse (LPN) at Oak Park Place is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
Responsibilities
- Provide thoughtful care to residents and their families based on the personalized Plan of Care
- Maintain accurate records and document changes in conditions and needs
- Proactively update the appropriate contacts on the plan of care, health changes, and concerns
Requirements
- Previous experience in nursing or other medical fields
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem-solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Must have a current LPN license
