Key Job Duties And Responsibilities

• Comply with the Facility’s personnel, safety, and corporate policies and procedures.
• Comply with the nursing department’s policies and procedures.
• Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager.
• Treat residents, family members, visitors, and team members with dignity and respect.
• Maintain the confidentiality of all resident and family information.
• Report to work on time and as scheduled.
• Absorb and assist in the admission and discharge of residents.
• Absorb and assist in daily hygiene care including but not limited to bathing (bed bath, shower, tub), oral, ear, eye, facial, hair and nail care.
• Keep resident’s dry – changing clothing and linen when it becomes soiled or wet.
• Absorb and assist in bowel and bladder functions; collecting specimens when required.
• Absorb and assist in transporting residents to/from appointments, activity, and social programs.
• Absorb and assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Report and, when appropriate, document, any change observed in a resident’s condition or behavior to the Charge Nurse.
• Absorb and assist in the care of dying residents; provide post-mortem care.
• Absorb and assist in administering professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, etc., as required.
• Absorb and assist in performing lab tests, blood pressures, and other routine monitored as required per the resident care plan.
• Absorb and assist in admitting, transferring, and discharging residents as required.
• Absorb and assist in completing accident/incident reports as necessary.
• Absorb and assist in completing and filing required recordkeeping forms/charts upon the resident's admission, transfer, and/or discharge.
• Absorb and assist in completing and charting nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care.
• Absorb and assist in filling out and completing accident/incident reports.
• Absorb and assist in reporting all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor.
• Absorb and assist in performing routine charting duties as required and in accordance with established charting and documentation policies and procedures.
• Absorb and assist in developing and maintaining a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents.
• Absorb and assist in completing required documentation of care and services as they are provided.
• Absorb and assist in reviewing complaints and grievances made or filed by your assigned personnel.
• Absorb and assist in making appropriate reports to the Nurse Supervisor as Licensed Practical Nurse Job Description / Rev 08.31.2022 Page 1 of 2 required or as may be necessary.
• Absorb and assist in following facility's established procedures.
• Absorb and assist in receiving/giving the nursing report upon reporting in and ending shift duty hours.
• Absorb and assist in reporting all occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
• Absorb and assist in monitoring your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies.
• Absorb and assist in recommending to the Nurse Supervisor the equipment and supply needs of your unit/shift.
• Absorb and assist in reviewing care plans daily to ensure that appropriate care is being rendered.
• Absorb and assist in maintaining the confidentiality of all resident care information.
• Absorb and assist in overseeing resident needs, staff and the operations, as needed.

Essential Job Duties/Physical Requirements

• Regular worksite attendance.
• Must have a valid professional license (LPN) that is in good standing with the State.
• Must have a valid CPR certification.
• Must have basic computer skills; must be able to read, write, speak, and understand the English language.
• Must be able to work beyond normal working hours and on weekends and holidays when necessary.
• Must be able to assist in the evacuation of residents during emergency situations.
• Must be able to perform the essential position functions of the job with, or without reasonable accommodation.
• Employee is required to use a transfer/gait belt or to obtain the assistance of another employee when attempting to assist residents when lifting or carrying objects in excess of 25 pounds.
• The employee must be aware that during the normal, routine performance of the essential functions, some of the following body movements may occur naturally, although hey may not be a requirement of the job.
