Licensed Practical Nurse

About the Role

The Licensed Practical Nurse provides direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations. This role ensures the highest degree of quality care is maintained under the supervision of the Director of Nursing Services or Nurse Supervisor.

Responsibilities

• Comply with facility, nursing department, personnel, safety, and corporate policies and procedures.
• Report allegations of resident abuse or neglect immediately to the Administrator, Director of Nursing, or Unit Manager.
• Treat residents, family members, visitors, and team members with dignity and respect; maintain confidentiality.
• Assist in the admission, transfer, and discharge of residents.
• Provide daily hygiene care, including bathing, oral care, hair and nail care, and assistance with bowel and bladder functions.
• Keep residents dry by changing clothing and linen when soiled or wet.
• Assist in transporting residents to appointments, activities, and social programs.
• Lift, turn, move, position, and transport residents using proper techniques and equipment.
• Report and document any changes in a resident’s condition or behavior to the Charge Nurse.
• Provide care for dying residents and perform post-mortem care.
• Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, and blood draws as required.
• Perform lab tests, blood pressure monitoring, and other routine tasks as outlined in the resident care plan.
• Complete accident/incident reports, medical forms, evaluations, and charting in an informative and descriptive manner.
• Ensure accurate and timely documentation of care, including nurses' notes and recordkeeping forms.
• Report discrepancies in physician’s orders, diet changes, or charting errors to the Nurse Supervisor.
• Maintain a good working rapport with inter-departmental personnel to ensure resident needs are met.
• Review complaints and grievances from assigned personnel and report to the Nurse Supervisor as needed.
• Receive and give nursing reports at the beginning and end of shifts.
• Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals per facility policies.
• Monitor assigned personnel to ensure compliance with safety regulations for equipment and supplies.
• Recommend equipment and supply needs for your unit/shift to the Nurse Supervisor.
• Review care plans daily to ensure appropriate care is being provided.
• Oversee resident needs, staff, and operations as needed.

Requirements

• Valid professional LPN license in good standing with the State.
• Valid CPR certification.
• Basic computer skills; ability to read, write, speak, and understand English.
• Ability to work beyond normal working hours, weekends, and holidays as necessary.
• Ability to assist in the evacuation of residents during emergencies.
• Ability to perform essential job functions with or without reasonable accommodation.

Physical Requirements

• Regular worksite attendance.
• Must be able to lift up to 10 lbs constantly, 11–25 lbs frequently, 26–50 lbs occasionally, and over 50 lbs occasionally.
• Must be able to carry up to 10 lbs constantly, 11–25 lbs occasionally, 26–50 lbs rarely, and over 50 lbs not required.
• Frequent reaching above shoulder height, at shoulder height, and below shoulder height.
• Constant pushing, pulling, grasping, handling, and fingering; frequent torquing.
• Twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing may be required.
• Ability to operate an automobile and various equipment as needed.
• Must be able to relate to and work with ill, disabled, elderly, emotionally upset, or hostile individuals.
• Must have adequate vision and hearing (or use prosthetics) to perform job requirements.
• Use of a transfer/gait belt or assistance from another employee when lifting or carrying objects over 25 lbs.
