Licensed Practical Nurse

About the Role

The Licensed Practical Nurse provides direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations governing the community. This role ensures the highest degree of quality care is maintained at all times under the direction of the RCC Services or Nurse Supervisor.

Responsibilities

• Comply with the community’s personnel, safety, and corporate policies and procedures, as well as nursing department policies.
• Report allegations of resident abuse or neglect immediately to the Executive Director, RCC, or Unit Manager.
• Treat residents, family members, visitors, and team members with dignity and respect while maintaining confidentiality.
• Assist in the admission, transfer, and discharge of residents.
• Provide daily hygiene care, including bathing, oral care, hair and nail care, and assistance with bowel and bladder functions.
• Assist residents with mobility, including lifting, turning, positioning, and transporting them using wheelchairs, lifts, and other equipment.
• Monitor and report changes in residents’ condition or behavior to the Charge Nurse.
• Administer professional services such as catheterization, tube feedings, suction, wound care, and colostomy care as required.
• Perform routine tasks like taking blood pressures, conducting lab tests, and documenting care in resident charts.
• Complete accident/incident reports, medical forms, and other administrative documentation as necessary.
• Chart nurses’ notes descriptively to reflect care provided and resident responses.
• Report discrepancies in physician’s orders, diet changes, or charting errors to the Nurse Supervisor.
• Develop and maintain good working relationships with inter-departmental personnel to ensure resident needs are met.
• Review and address complaints or grievances from assigned personnel.
• Receive and give nursing reports at the beginning and end of shifts.
• Report occupational exposures to blood, body fluids, or hazardous materials per community policies.
• Monitor assigned personnel to ensure compliance with safety regulations.
• Recommend equipment and supply needs for the unit/shift to the Nurse Supervisor.
• Review care plans daily to ensure appropriate care is being provided.
• Oversee resident needs, staff, and operations as needed.

Requirements

• Valid professional LPN license in good standing with the state.
• Basic computer skills and proficiency in reading, writing, speaking, and understanding English.
• Ability to work beyond normal hours, including weekends and holidays, as necessary.
• Ability to assist in resident evacuations during emergencies.
• Capability to perform essential job functions with or without reasonable accommodation.

Physical Requirements

• Regular attendance at the worksite.
• Frequent lifting (up to 25 lbs), occasional lifting (26–50 lbs), and constant carrying (up to 10 lbs).
• Constant reaching at or below shoulder height; frequent reaching above shoulder height.
• Constant pushing/pulling and hand manipulation (grasping, handling, torquing, fingering).
• Frequent twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing.
• Ability to operate an automobile and assist with various physical activities.
• Ability to work with ill, disabled, elderly, emotionally upset, or hostile individuals.
• Sufficient vision and hearing (with or without prosthetics) to perform job requirements.
