Responsibilities
- Serve as a trusted clinical resource for clients receiving psychiatric services.
- Conduct effective telephone and in-person triage while making sound clinical decisions within your scope of practice.
- Identify medical concerns, medication side effects, and health risks before they become larger problems.
- Cookordinate care with primary care providers, specialists, hospitals, community partners, and caregivers to improve client outcomes.
- Help clients successfully manage chronic conditions such as diabetes, COPD, and Hepatitis through education, monitoring, and skills training.
- Administer medications and injections accurately and safely.
- Support psychiatric providers by ensuring critical medical information is communicated and acted upon appropriately.
- Maintain accurate clinical documentation within required timelines.
- Contribute to a collaborative, recovery-oriented treatment environment.
Requirements
- Active Oregon LPN license.
- Minimum of two years of professional nursing experience.
- Valid Oregon driver's license and acceptable driving record.
- Ability to travel throughout the community and client service area.
Qualifications
- Preferred: Experience in behavioral health, psychiatric nursing, community mental health, or integrated healthcare settings.
- Preferred: Experience working with adult populations.
- Preferred: Knowledge of psychotropic medications and medication monitoring.
- Preferred: Experience coordinating care across multiple healthcare systems.
