About the role
We are seeking talented, passionate individuals to join our team and help our patients live happier and healthier lives in their homes.
Responsibilities
- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes by conducting patient home visits.
- Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs.
- Manage emergency situations in a prompt, precise, and professional manner.
- Complete medication administration and treatment needs for all assigned patients.
- Provide nursing care in accordance with applicable regulations and Company policy.
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
Requirements
- Active LPN state license.
- Valid driver’s license.
- Two or more years of applicable nursing experience.
