Licensed Practical Nurse

About the role

Commonwealth Home Health Network is looking for Licensed Practical Nurses to join our team. CHHN provides in-home, skilled and unskilled services to seniors, older adults, and those who are disabled.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provide skilled nursing care/services in accordance with prescribed orders.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collect, analyze, and interpret data and information from health care members and document actual and/or potential nursing diagnoses.

Qualifications

• Valid nursing license (required)
• 1+ years of licensed practical nursing experience
• Associate degree in nursing (required)
• Home health experience is a plus
