About the Role

In a team relationship with the Registered Nurse, the Licensed Practical Nurse contributes to the nursing process through on-going systematic data collection, assisting with the health assessment, recording observations, goal identification, and patient health education.

Responsibilities

  • Collaborates with the healthcare team to support patient care.
  • Initiates the admission process and assists with identifying recurrent health problems, symptoms, and behavioral changes.
  • Collects pertinent data through observation, interview, and other members of the health team; communicates changes in patient condition to care providers.
  • Records all assessment data in the patient’s health record.
  • Assists in forming a written care plan, including short- and long-term measurable goals with anticipated behavioral changes.
  • Assists in setting priorities when planning care to meet patient needs.
  • Participates in multi-disciplinary care conferences.
  • Counts CDS drugs and contraband, if required.
  • Assists physicians in minor medical/surgical procedures.
  • Transcribes and posts all therapeutic orders generated by the physician.
  • Initiates and documents prescribed treatments and nursing interventions in compliance with policy.
  • Administers routine and/or PRN medications as ordered.
  • Practices universal precautions when providing nursing procedures.
  • Assists in maintaining a neat, clean, and safe environment for patients and co-workers.
  • Keeps patient information confidential and respects patients’ right to privacy.
  • Informs the health team of observations and changing patient conditions.
  • Maintains and provides bi-annual proof of active state nursing licensure.
  • Adheres to nursing code policy, including wearing a white lab coat.
  • Arrives promptly for scheduled shifts and participates in verbal report handoffs.
  • Notifies the RN Supervisor if ill while on duty and provides a doctor’s excuse if absent before or after scheduled days off, if requested.
  • Submits vacation or time-off requests in advance of schedule posting.
  • Arranges shift exchanges within the same pay period and notifies the Charge Nurse/RN Supervisor in writing of any changes.

Qualifications

  • Graduate of an accredited Licensed Practical Nurse program with current licensure in the state of employment.
  • Must conform to the security clearance standards of the employing institution.

Functional Demands

  • Ability to pass all required health and other screening tests.
  • May be required to work nights, weekends, and holidays.
  • On-call duty may be required (available to work on short notice).

  • Participates in ongoing quality improvement activities.
  • Completes required orientation as directed by the facility.
  • Follows facility and OSHA safety rules and procedures while on assignment.
  • Follows facility and CFG Health Systems Occurrence Protocol.
  • Upholds HIPAA and PREA regulations.
  • Punctual and dependable for assigned/confirmed shifts.
  • Maintains security clearance for the assigned facility.
  • Conforms to dress code and reports to work site at designated time.
  • Adheres to attendance policy and utilizes timekeeping practices and expectations.

Schedule

Full-time night shift: 7:00 PM – 7:30 AM.
