LICENSED PRACTICAL NURSE

About the role

We provide medication management and general health and wellness support in a residential setting. We collaborate with other providers to facilitate client wellness and maintain confidentiality.

Responsibilities

  • Provides medication management and administration to clients in the residential environment.
  • Documents medication administration in each client’s Medication Administration Record.
  • Tracks client compliance with required medications and reports concerns to other Prescribers and medical staff.
  • Coordinates shift coverage for unapproved or unplanned absences.
  • Supports client wellness by observing and identifying potential medical or medication concerns.
  • Communicates and collaborates with appropriate staff and providers to coordinate client care and services.
  • Completes required paperwork and facilitates transportation to medical appointments.
  • Promotes health and wellness within the residential facility and assists with client wellness education and activities.
  • Supports medical staff and providers by providing assistance with vital signs and other tasks.
  • Collaborates with residential staff on client discharge plans.
  • Attends staff meetings and medical/clinical supervision as directed.
  • Participates in relevant Agency meetings and/or trainings.
  • Develops professional competencies related to job role performance through learning, supervision, in-service training, and conferences/workshops.

Qualifications

  • High School Diploma or GED and accredited LPN diploma required.
  • Valid Licensed Practical Nurse required.
  • Two to three years of experience related to nursing services to individuals with mental health, alcoholism, chemical dependency, substance use disorder related services preferred.
  • Bi-lingual in Spanish preferred.

Requirements

  • Frequent sitting, talking, hearing, standing, walking, using hands to type, and performing light lifting.
  • Close vision, distance vision, and ability to adjust focus.
  • Quiet noise level in the work environment.
  • Valid NYS Driver’s license and use of a registered and reliable vehicle.
