About the role

Under the direction and supervision of the Clinic Practice Manager, administer medications, implement primary care orders, and evaluate participant complaints physically and/or by telephone/telehealth. Provide education to participants and caregivers regarding care plans, nursing treatments, or medications. Communicate participants’ conditions to primary caregivers, physicians, and the interdisciplinary team.

Responsibilities

  • Perform basic nursing services in the PACE clinic within scope of practice as delegated by the Clinic Practice Manager.
  • Participate in planning and executing interventions in accordance with participants' plans of care in coordination with the Interdisciplinary Team and the RN Care Manager.
  • Administer prescribed medications, vaccinations, and treatments in accordance with nursing standards and document appropriately.
  • Withdraw blood from participants as instructed, when designated by the Clinic Practice Manager and within scope of practice.
  • Maintain participants’ medical records and fulfill PACE organization charting and reporting requirements.
  • Observe, record, and report participants’ conditions and reactions to drugs and treatments to physicians.
  • Triage participants within scope of practice to determine priority of care.
  • Educate participants and family/caregivers regarding medications and treatment instructions.
  • Prepare equipment and aid physicians during treatment, examination, and testing of participants.
  • Assist other staff in clinic with daily appointments and emergencies, maintaining timely flow of participants.
  • Respond to and refer incoming telephone calls.
  • Assist in maintaining exam rooms for necessary supplies and materials, ensuring cleanliness.
  • Ensure clinic supplies are on the reorder list as appropriate and monitor expiration dates of medications and supplies monthly.
  • Communicate effectively in interdisciplinary team meetings, clinic meetings, or any other assigned meetings.
  • Assist in scheduling specialist appointments, hospital admissions, and calling reports to units as assigned.
  • Follow all BoldAge Policies and Procedures and OSHA safety guidelines.
  • Maintain confidentiality of all company procedures, results, and information about participants, clients, or families.
  • Handle potentially infectious specimens with appropriate biohazard precautions and practice Universal precautions.
  • Maintain a safe working environment and follow BoldAge Safety Policies and Procedures.
  • Participate in and support Quality Improvement initiatives.
  • Attend continuing education classes and required staff and training meetings.
  • Maintain professional affiliations and any required certifications.
  • Perform other duties as required or requested.

Requirements

  • Graduate of an accredited school of nursing.
  • Current LPN/LVN license in good standing to practice in the required state (NJ: New Jersey State Board of Nursing).
  • BLS certification required (must obtain within 90 days of employment).
  • IV Certification preferred.
  • One year of experience working as an LPN strongly preferred.
  • One year of experience working with a frail or elderly population preferred; training will be provided if not present.

Pre-employment requirements

  • Must have reliable transportation, a valid driver’s license, and the minimum state-required liability auto insurance.
  • Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Must pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
