Position Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide dedicated 1:1 clinical support. In this role, you will be the primary medical point of contact, ensuring safety, and helping the individual achieve their daily goals. The LPN will provide 1:1 individualized nursing support, including during transportation to and from Turners Falls to Baroco located in Hadley, MA. The LPN works under the supervision of the Health Care Supervisor (RN) and Program Director.

Responsibilities

  • Provide 1:1 direct care and assist with ADLs, feeding, and mobility
  • Perform skills/treatments required for consumer health (i.e. G-Tube feedings, tracheotomy care)
  • Administer medications and treatments per provider orders
  • Observe and report changes in health status to the RN, supervisors, and the individual's family/guardian
  • Complete daily, weekly, monthly, and required nursing documentation per MassHealth Regulations
  • Maintain first aid supplies, equipment, and medication inventory
  • Advocate for participant rights, safety, health, and independence
  • Practice within LPN scope and seek clarification/training when needed
  • Participate in trainings, meetings, and ongoing professional development

Qualifications

  • Valid Massachusetts LPN license
  • High school diploma or equivalent (minimum)
  • Valid driver's license + reliable transportation (must meet insurance criteria)
  • Ability to lift up to 60lbs and assist with transfers
  • Strong communication, teamwork, and problem-solving skills
  • Experience with individuals with developmental disabilities preferred
