We are seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide high-quality nursing services within a community-based setting for individuals with intellectual, developmental, psychiatric, and behavioral needs. Responsibilities

• monitoring health conditions and ensuring quality care
• administering medications and supervising staff in medication administration
• conducting assessments and implementing healthcare plans
• providing staff training on infection control, medication, and safety protocols
• acting as a liaison with medical and service providers
• training individuals on self-care topics such as medication use, STD prevention, and smoking cessation
• providing medical treatment, overseeing staff, and supporting individuals' health and independence

Qualifications

• Associate’s Degree in Nursing or related field
• current state LPN or LVN licensure
• at least one year of relevant experience
• valid driver’s license and certifications such as CPR/FA
• strong communication skills with staff, individuals served, and families
