Role Overview: The licensed practical nurse (LPN) provides direct patient care under supervision, supporting registered nurses and healthcare providers. Key Responsibilities Collects patient data through observation and communicates findings to the care team.Delivers care aligned with the patient care plan, policies, and relationship-based care principles.Participates in team activities, quality improvement projects, and mentoring. Qualifications & Skills Graduate of an accredited practical nursing program or has passed the LPN Boards.One year of LPN experience in applicable settings preferred.Excellent verbal and written communication skills.Experience working in team environments and computer literacy, with prior EMR experience preferred.Current LPN license per state requirements and BLS certification if applicable.
