About the role
This is a full-time position (30–40 hours per week) for a Licensed Physical Therapist Assistant (PTA) at a top-rated Continuing Care Retirement Community in North Manchester, Indiana, about 40 miles southwest of Fort Wayne. The community offers a full continuum of care including independent living, assisted living, memory care, skilled nursing, rehab-to-home, and has a growing community-based outpatient therapy program.
Responsibilities
- Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.
- Maintain accurate documentation of therapy sessions and resident programs.
Requirements
- Active Indiana Physical Therapist Assistant (PTA) license or eligibility to obtain one.
- Graduation from a CAPTE-accredited physical therapist assistant education program.
- Passing of a state-administered national exam.
- Fulfillment of Indiana’s requirements for licensure.
- Legal eligibility to work in the United States (proof required upon offer).
