Responsibilities

  • Implement treatment plans developed by the supervising Physical Therapist.
  • Absorb therapeutic exercises, gait training, and transfer techniques.
  • Support mobility, balance, and strength-building activities.
  • Educate patients and caregivers according to the established care plan.
  • Observe, document, and report patient progress or changes in condition.
  • Maintain accurate and timely clinical documentation.
  • Collaborate with Physical Therapists and interdisciplinary care team members.
  • Follow agency policies, clinical standards, safety protocols, and applicable home health regulations.

Qualifications

  • Active Illinois Physical Therapist Assistant (PTA) license/certification required.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Prior experience working as a Physical Therapist Assistant (PTA) is required.
  • Home health experience preferred, but not required.
  • Current CPR certification.
  • Strong communication, organizational, and time management skills.
  • Ability to work independently in a home health setting.
  • Valid driver's license and reliable transportation for travel throughout the Chicago service area.
