Responsibilities
- Implement treatment plans developed by the supervising Physical Therapist.
- Absorb therapeutic exercises, gait training, and transfer techniques.
- Support mobility, balance, and strength-building activities.
- Educate patients and caregivers according to the established care plan.
- Observe, document, and report patient progress or changes in condition.
- Maintain accurate and timely clinical documentation.
- Collaborate with Physical Therapists and interdisciplinary care team members.
- Follow agency policies, clinical standards, safety protocols, and applicable home health regulations.
Qualifications
- Active Illinois Physical Therapist Assistant (PTA) license/certification required.
- Graduate of an accredited Physical Therapist Assistant program.
- Prior experience working as a Physical Therapist Assistant (PTA) is required.
- Home health experience preferred, but not required.
- Current CPR certification.
- Strong communication, organizational, and time management skills.
- Ability to work independently in a home health setting.
- Valid driver's license and reliable transportation for travel throughout the Chicago service area.
