About the role
This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner.
Responsibilities
- Demonstrates proficiency with assessment skills by completing psychosocial assessments at intake and in ongoing intervals as outlined by program requirements; completing standardized assessments and/or outcome-based measures.
- Develops individualized treatment plans by adhering to applicable county, state and federal standard regulations when developing individualized treatment plans for consumers.
- Provides therapy to consumers in individually recommended setting by providing therapy in recommended setting by adhering to recommendations outlined in individual consumer treatment plans.
- Provides treatment of the whole person by providing service in collaboration with all team members involved with the consumer, and with the understanding that treatment of the whole person is crucial.
- Evaluates individualized consumer-driven treatment plan reviews by completing ongoing evaluation of efficacy of treatment evaluations, via development of treatment plan reviews.
- Develops transition and discharge planning by evaluating recovery and progress through treatment goals, symptomology.
- Completes all required clinical documentation by completing progress notes per service, including necessary signatures, discharge summaries, incident reports, treatment reviews via electronic health record system.
- Responds to emergency/crisis situations with team members and consumers by committing to ensuring the safety of the consumer, and the proper utilization of de-escalation skills, when needed.
- Maintains assigned, planned schedule by following program guidelines to ensure regular service provision and maintenance of caseload, per agreement with program supervisor.
- Maintains coordination of care through routine conversations and referrals when needed (i.e., natural supports identified by the consumer, outside referrals, PCPs, school districts, etc.).
- Maintains a commitment to professional development by participating in ongoing supervision, training, and department and team meetings, as outlined by policies and procedures.
Requirements
- Has a bachelor’s degree from an accredited college or university in Psychology, Social Work, nursing, rehabilitation or activity program, or has a graduate degree in a clinical discipline with 12 graduate level credits in mental health or counseling.
- At least 1 year of experience with either a CASSP service system, or employed by a licensed mental health services agency, or subcontracted agency, or other mental health clinicians with documents training in the field of behavioral modification techniques.
Qualifications
- Ability to read, analyze, and interpret complex documents and instructions.
- Ability to respond effectively to sensitive inquiries or complaints.
- Ability to clearly explain instructions in both written and oral media.
- Basic mathematic ability including addition, subtraction, multiplication, and division.
- Use of MS Excel to aid in mathematical work.
- Sound decision-making ability to support consumers in a calm and timely manner.
- Ability to utilize a variety of therapeutic modalities based upon consumer needs, and react quickly to prevent consumer injury or injury to another due to consumer behavior.
