Licensed Nurse, LPN - Home Health - PRN - Muskogee

About the role

VitalCaring is a company dedicated to providing care in patients' homes. As a Home Health LPN/LVN, you will play a crucial role in helping patients recover and maintain independence.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and support the plan of care developed by the RN
• Deliver skilled nursing services, including treatments, medication support, and wound care
• Maintain infection control and proper use of equipment and supplies
• Document patient care accurately and timely within the EMR
• Communicate effectively with physicians, nurses, and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing training
• Sustain coordinated care by maintaining consistent communication across the care team

Requirements

• Active LPN/LVN license in the state of practice
• Graduate of an accredited practical/vocational nursing program
• CPR certification
• Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)

Qualifications

• Strong clinical and observational skills
• Clear communication and patient education abilities
• Basic proficiency with EMR systems

Skills

• Experience in home health or community-based care
• Strong patient engagement and relationship-building skills
• Ability to work independently while staying connected to a team
• Familiarity with Homecare Homebase (HCHB)
