Licensed Nurse

Responsibilities

• Affords assistance with medications, diagnostic test orders, and correspondence as directed by the provider.
• Maintains oversight of lab and diagnostic test results to ensure high-quality patient care.
• Facilitates referrals to appropriate professionals or external agencies as recommended by the provider.
• Coordinates with pharmaceutical representatives, schedules in-services, and educates staff on new medication information.
• Maintains sign-in sheets, provides health education to clients and their families, and manages the medication holding area.
• Ensures proper handling and disposal of expired medications, checks in and out medications according to pharmacy board guidelines, and coordinates with pharmacies for adherence to medication regimens.
• Facilitates access to medications through the Central Pharmacy, patient assistance programs, and medication samples, and supervises the filling of medication boxes on-site.
• Administers injections as prescribed, provides triage for client symptoms and side effects, and serves as a liaison between clients and providers.
• Conducts health history assessments for each client, annually, and participates in interventions with a multi-disciplinary treatment team.
• Completes clinical documentation in the ECR, assesses program needs, and cooperates with performance improvement objectives.
• Complies with confidentiality and protected health information rules, attends staff meetings, in-services, case conferences, and performs other related duties as assigned.
• Supports the agency by performing tasks such as reminder calls, vital signs monitoring, patient and family expectations, informed consent for telehealth, CSA, OARRS, prior authorizations, maintaining the white board, checking clients in and out for providers, uploading Genesight results to ECR, faxing, scanning, and copying.
• Collaborates in maintaining a clean, safe workplace, including maintaining CPR/First Aid skills and aggressive client management skills (TAPS or equivalent), and continuing education.
• Manages Vivitrol coordination, referrals, and spreadsheets, and may be called to work at other locations.
• Works flexible hours based on provider availability, with productivity expectation of 30%+.

Requirements

• Independent licensure (RN, LPN)
• Competency in areas necessary for performing the activities outlined above
• Good customer service skills with clients and the public at large
• Uses computer to perform clinical documentation in Carelogic (MIS system) and generate written communication in WORD or via email
• Completes Relias Training as scheduled
• Physical demands include standing, walking, sitting, talking, hearing, lifting up to 10 pounds, and occasional bending, reaching, and stooping.
• The employee must have a valid driver's license and a safe driving record.
• The employee must be comfortable driving in varied weather conditions and be exposed to latex.
