About the role

Connections Health Solutions is a leading provider of immediate-access behavioral health crisis care. Our team combines medical and recovery-oriented treatment to stabilize individuals in crisis and connect them to community-based resources for ongoing recovery.

Responsibilities

  • Facilitate admission intakes, educating and informing individuals about consents for treatment and authorizations for use and disclosures.
  • Conduct a thorough, accurate, and clinically sound Psychosocial Assessment with the individual within 72 hours of admission to Walk-In or observation, and within 24 hours for individuals admitted to the CSU.
  • Maintain the clinic record, including documentation of activities performed as part of the service delivery process, and obtain signatures from individuals regarding treatment, as necessary.
  • Collaborate with the interdisciplinary treatment team to develop and implement individualized, person-centered Treatment Plans suitable for brief intervention.
  • Complete and review signed Treatment Plans on CSU with each individual and any involved parties to ensure understanding and agreement.
  • Gather collateral information from individuals and involved parties to conduct a thorough assessment of needs for treatment and discharge planning.
  • Actively coordinate care and provide referrals to ensure services are aligned with both facility staff and external providers.
  • Identify and address gaps in service needs, making appropriate recommendations to meet individuals' needs upon discharge.
  • Collaborate with the interdisciplinary treatment team to engage with, monitor, and communicate with individuals, ensuring safety and meeting their needs during the discharge process.
  • Maintain accurate clinic records, discharge planning, and treatment planning.
  • Maintain a working knowledge of the Pennsylvania State behavioral health system and 302 involuntary legal processes.
  • Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.
  • Demonstrate competency through post-testing, skill observation, and performance assessments as conducted by direct supervisors.
  • Perform all other duties as assigned.

Requirements

  • Master’s degree in Social Work, Counseling, Marriage and Family Therapy or a related behavioral health field, and current Pennsylvania licensure in good standing as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC).
  • 1-2 years of experience in a mental health setting.
  • BLS (Basic Life Support) must be obtained within seven days of hire.

Qualifications

  • Experience in acute mental health crisis settings is a plus.

Skills

  • Knowledge of the Pennsylvania State behavioral health system and 302 involuntary legal processes.
  • Ability to collaborate with interdisciplinary treatment teams.
  • Strong communication and interpersonal skills.
  • Proficiency in clinical assessment and treatment planning.

Pay

  • Maximize Your Earnings With Additional Hourly Pay +$2/hr -> Monday-Thursday Nights+$3/hr -> Friday-Sunday Nights+$2/hr -> Saturday-Sunday Days

Schedule

  • Full-time only.
