About the role
Perform a comprehensive list of psychotherapy functions and practices. Work with internal and external stakeholders to promote quality patient-centered behavioral health care.
Responsibilities
Assessment
- Identifies potential barriers and current difficulties in participating in treatment at intake and throughout the course of treatment.
- Appropriately utilizes screening and assessment tools approved by CPH including but not limited to CSSR-S, PHQ-9, Modified mini, substance use and risk assessment.
- Interprets quantitative and qualitative data within the social, political, economic and cultural context to determine patient-centered action plan.
- Evaluates across the bio-psycho-social continuum including environmental factors and patient resources.
- Builds rapport and demonstrates a safe environment in which patients engage in the comprehensive assessment process.
- Perform ongoing continuous assessments to develop and maintain appropriate, safe, strengths-based and person-centered care.
- Utilize diagnostic criteria, symptoms, differential diagnosis, and rule outs to determine accurate diagnosis.
Individual Counseling
- Responsible for an individual caseload including 1:1 counseling, treatment, relapse prevention, discharge planning, care coordination and referral.
- Participate in case conferences and clinical meetings weekly.
- Provide individualized patient care utilizing best practice counseling skills, techniques and modalities to improve quality of life and reduce symptoms.
- Utilize crisis intervention skills and community resources to promote patient safety.
- Utilize trauma-informed approaches including taking universal precautions.
- Engage in ongoing professional maintenance activities to include but not limited to clinical supervision, continuing education, and counselor wellness.
Group/Family Counseling and Education
- Development, execution and documentation of group therapy on assigned topics to be determined by patient need and counselor skill set.
- Awareness of group dynamics, ability to manage time and destructive behaviors, encourage participation and engage patients in the learning process.
- Conduct family, marriage and/or couples therapy as clinically indicated.
- Provide psychoeducation to individuals, groups and families on topics such as substance use disorders, mental health and wellness, lifestyle changes, coping skills, and recovery.
- Strengthen relationships among people in a purposeful effort to promote, restore, maintain, and enhance the well-being of individuals, families, social groups, organizations, and communities.
Documentation
- Document in a variety of manners including assessments, individual and group notes, collaboration, collateral contact with internal and external stakeholders to the patient.
- Treatment planning, discharge planning and regularly updating the patient record to demonstrate patient’s progress or lack thereof in the treatment environment.
- Assessments are performed per specific scope and practice at or above standards set by OASAS, OMH, TJC and DNV.
- Forms are filled out respecting the various deadlines required by the governing and regulatory bodies and will remain in the medical record.
- Confidential maintenance of and transmission of medical records following HIPAA and CFR 42 guidelines.
Clinical Supervision
- Provide clinical supervision for interns and/or junior staff of the same discipline.
- Act as a mentor, coach and evaluator to create an atmosphere that promotes counselor self-reflection, discipline, motivation, learning, wellness and professional development.
- Monitor quality of professional services, attend to ethical issues and ensure quality clinical care.
- Act as a resource and consultant for other departments as needed.
- Other duties as assigned.
Qualifications
Preferred Qualifications
- Individual and/or group counseling experience.
- Two years of mental health experience.
Minimum Qualifications
- Master's Degree in Human Services field.
Licensure / Certifications
LCSW, LMSW, LMHC, LCAT, LMFT or permit holder in one of these professions required. Limited permit or license required within 1 year of hire.
Physical Requirements
Sedentary Work – Exerting up to 10 pounds of force occasionally. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.
Pay
Pay Range: $62,400.00 – $75,300.00
