Licensed Medical Social Worker
Responsibilities
- Conduct psychosocial assessments to evaluate patient needs.
- Provide brief counseling and crisis intervention to patients and families.
- Coordinate discharge planning to ensure smooth transitions of care.
- Connect patients and families with community resources for support.
- Collaborate with an interdisciplinary team of healthcare professionals, including physicians, nurses, and therapists.
- Advocate for patient needs and assist with advance care planning.
