Responsibilities

  • Provide crisis intervention, service linkages, and discharge planning.
  • Develop treatment plans with individuals and the treatment team; monitor and evaluate progress; modify treatment plans as needed.
  • Determine discharge needs and appropriate community resources.
  • Establish and maintain relationships with community providers to ensure continuity of care.

Minimum Qualifications

  • Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW). Candidates may be considered under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program.
  • Alternatively, candidates within State Service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Shift Differential Pay

  • 7% differential for evening and weekend day shifts.
  • 10% differential for night shifts.

Additional Information

This position is located at Central New York Psychiatric Center's Groveland Satellite Unit on the grounds of Groveland Correctional Facility in Livingston County.

Employment is contingent upon maintaining an active license and registration. Failure to do so may result in removal from the position.

Candidates must not be listed on any Federal or State Medicaid and Medicare exclusion lists, nor be excluded from any other Federal or federally assisted program. If an employee becomes listed on such an exclusion list, termination may occur.

Appointees may be required to become enrolled Medicare providers, obtain a National Provider Identifier (NPI) number, and actively participate in billing through Medicare and Medicaid. Loss of billing eligibility may result in termination.
