About the Role
The Home Health Licensed Master Social Worker is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation. The primary function is to provide coordinated care in the home to clients of all age groups, perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient and family.
Responsibilities
- Provides care utilizing infection control measures that protect both staff and patients (OSHA).
- Demonstrates commitment, professional growth, and competency by attending in-services as required by regulation and participating in in-service programs.
- Accepts clinical assignments consistent with education and competence to care for patients.
- Monitors assigned cases to ensure compliance with requirements of third-party payors.
- Provides effective communication to patient/family, team members, physicians, and other healthcare professionals to ensure coordination of services; provides appropriate referrals as necessary.
- Ensures continuity of quality patient care delivered according to the plan of care with appropriate documentation.
- Evaluates and assesses home health patients to identify psychosocial (counseling/intervention), financial, environmental, and community needs, including the Plan of Care (POC), documentation, clinical records (clinical and progress notes), case conferences, and community resource referrals.
- Evaluates effectiveness and outcomes of care.
- Performs other duties as assigned.
Requirements
- Master’s degree in Social Work required.
- One (1) year of experience as a Social Worker in a healthcare agency preferred.
- Active and unencumbered Licensed Master Social Worker in the state(s) of employment required.
- State-Specific Requirements: Florida licensure required to be titled “social worker” or practice social work (LBSW, LMSW, LCSW, or LICSW).
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Ability to understand, read, write, and speak English.
- Articulates and embraces hospice philosophy.
Physical Demands and Work Environment
The demands of this role necessitate regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
