Requirements

• Valid driver’s license and reliable transportation
• Flexible schedule to accommodate client needs

Qualifications

• LMSW or LCSW license
• Experience with older adults, care management, or related fields
• Knowledge of Medicare, Medicaid, and community services
• Strong interpersonal and advocacy skills
• Proficiency with generic office software

Pay

$70,000–$75,000 per year
