Licensed Independent Clinical Social Worker -

Responsibilities

• Evaluate patients mental and physical health, addiction, or problematic behavior and assess their readiness for treatment
• Utilize various therapeutic models and theories to understand and plan intervention strategies for patients
• Complete screening, intakes, assessments, treatment plans and discharge summary as needed throughout course of patient care
• Maintain patient records in our electronic medical record system
• Develop, recommend, and review treatment goals and plans with patients and their families
• Absorb and educate patients about community-based organizations such as Alcoholics Anonymous and social and employment services that may help restore them to productive roles in the community
• Monitor patient progress and properly maintain patient records in our electronic medical record system
• Collaborate with other community agencies as necessary for optimal patient care
• Attend monthly multi-disciplinary team meetings to discuss progress and/or barriers to patient care
• Use a combination of individual and group counseling sessions to help patients and observe their attitudes and readiness to accept help and guidance
• Maintain case histories and progress reports. Modify treatment programs based on observation
• Absorb and educate patients about community-based organizations such as Alcoholics Anonymous and social and employment services that may help restore them to productive roles in the community

Requirements

• Minimum of 2 years work experience in the chemical dependency field
• Masters degree
