About the Role

The Care Coordinator applies clinical expertise to manage care for medically complex members, ensuring access to appropriate services across various settings.

Responsibilities

• Conduct assessments and develop personalized care plans.
• Educate members and facilitate referrals to community resources.
• Advocate for members to overcome barriers to care.
• Collaborate with care teams and monitor hospital encounters.
• Participate in quality improvement initiatives.
• Use health plan software for documentation and compliance, ensuring timely and accurate record-keeping.
• Perform field-based duties, including member and provider visits, with travel within assigned territories.

Requirements

• Active RN license.
• 2+ years of clinical experience.
• Proficiency in MS Office.
• Strong organization, communication, and time management skills.
• Must reside near specified counties.
• Reliable transportation and ability to travel up to 75%.

Preferred Qualifications

• BSN or higher degree.
• CCM certification.
• Community case management experience.
• Knowledge of Medicare/Medicaid regulations.
