About the Role
Part-time, daytime hours at Rockford Center, a 138-bed mental health facility in Newark, DE. Rockford Center provides inpatient and partial behavioral health programs for children, adolescents, adults, and older adults, and is the only child inpatient behavioral health facility in Delaware. The center serves New Castle County and northern Maryland, with an experienced senior management team.
Responsibilities
- Assess nutritional status and provide individualized nutrition care plans and education tailored to patient needs.
- Analyze medical records and conduct face-to-face interviews with hospitalized patients using specialized knowledge and skills.
- Document findings, actions, and summary assessments, and follow up on diet instructions and comprehension in the DCH electronic medical record system.
- Educate and counsel patients of all ages, including caregivers, on dietary modifications required for continued recovery.
- Review and modify patient room service meal selections as needed, communicating with the Food and Nutrition Departments and providers.
- Attend interdisciplinary team patient rounds.
- Provide information to the medical team, including doctors, nurses, physical therapists, and pharmacists.
- Perform assessments and document individualized care plans using remote computer access during off hours when needed.
Requirements
- Must be a licensed dietitian in the State of Delaware.
- Certified diabetes educator preferred.
- Two (2) years of related experience, with at least one (1) year of clinical setting experience preferred.
- CPR certification required.
- Completion of Handle with Care training within 90 days of employment.
