Licensed Clinician - (LCSW, LCPC, LMFT)- Casey, IL

About the Role

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) to set a new bar for the quality of care provided to seniors. This autonomous role allows clinicians to function like a private practice without the hassle of managing a back office, with a 40-hour work week primarily focused on clinical work and patient documentation.

As an on-site clinical provider, you will work with residents and their families as an active, lead member of an interdisciplinary team addressing emotional, social, and psychological needs. You will manage your caseload, schedule, and salary while providing initial assessments, psychotherapy, and ongoing support.

Responsibilities

• Provide initial assessments and utilize psychotherapy to address mental health issues.
• Offer ongoing support to residents and their families.
• Function as a lead member of an interdisciplinary team of physicians and nurse practitioners.
• Manage all documentation requirements and stay up-to-date on Medicare Benefit and commercial insurance plans.
• Collaborate with interdisciplinary teams, including other MPAC providers, to ensure optimal patient care.
• Foster open communication with facility teams and Regional Clinical Managers to ensure the success of clinical programs.
• Take initiative and communicate effectively with patients, families, nursing staff, and other stakeholders.
• Maintain high organizational standards to manage patient needs and ensure mental health requirements are met.

Requirements

• Master’s degree and a valid LCSW, LMFT, or LCPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Demonstrated knowledge of mental health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams.
• Ability to work as a team member with a positive attitude.
• Strong communication skills in multiple settings (patients, families, nursing staff, etc.).
• High level of organization to manage patient needs and documentation.
• Comfortable working in the dynamic environment of a skilled nursing facility.

Additional Qualifications

• Knowledge of Medicare Benefit and commercial insurance plans.
• Experience in a healthcare setting, particularly in nursing homes (preferred).
• Passion for working with older adults.
• Exceptional organizational skills.
