Licensed Clinical Social Worker, Remote

Responsibilities

• Provide case consultation and collaborate with interdisciplinary care teams
• Conduct case management, including assessing patient needs, coordinating care, and connecting patients with community resources
• Lead group sessions focused on psychoeducation, stress management, mindfulness, and meditation
• Conduct one-on-one patient visits to provide emotional support, stress management techniques, psychoeducation, and coping strategies
• Document patient encounters and coordinating follow-up care to support positive health outcomes

Requirements

• LCSW license with licensure to practice in GA, FL, and MI (also open to PA, NY, OH)
• Bachelor’s and Master’s degrees required
• Experience communicating with patients asynchronously preferred
• Experience with GI conditions or weight management a plus
• Proficiency in using telehealth platforms and electronic health record (EHR) systems
• Experience working with patients in a clinic setting preferred
• Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease
• Strong personal computing skills - remote work experience preferred
