Licensed Clinical Social Worker / PRN
About the Role
We are seeking an experienced Licensed Clinical Social Worker (LCSW) passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery. You will deliver therapeutic interventions and resource recommendations to help patients achieve personal, career, and independent living goals in the most integrated setting possible. Professional autonomy, teamwork, communication, and collaboration are key to maximizing patient recovery.
Responsibilities
- Performs comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
- Sets measurable objectives in conjunction with the rehab team.
- Assists physicians and other health team members in understanding significant social and emotional factors related to the patient's health problems.
- Instructs and counsels patients and families on treating and coping with social and emotional responses connected to illnesses.
- Participates in care coordination activities and acts as a resource to other health team members in identifying and resolving patient needs.
- Identifies and assists patients and/or families in utilizing appropriate community resources to achieve measurable objectives.
Requirements
- Master's Degree in Clinical Social Work (LCSW).
- Current Licensed Clinical Social Worker (LCSW) licensure in the state of practice.
- Minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis.
- Current BLS (Basic Life Support), First Aid, and CPR certification.
- Ability to lift 50 pounds; may involve lifting up to 100 pounds following safety procedures.
- Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
- Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
