We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting—a person's own home and community.

We are looking for experienced Licensed Clinical Social Workers to provide truly patient-centered care in Temecula, CA and surrounding areas. PRN positions are available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans practiced in patients' homes and communities. Therapists are creative in their approach, incorporating a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Responsibilities

  • Assess, counsel, and direct medically complex patients by developing individualized treatment plans
  • Assist patients with adjustment to disability, resource linkage, and behavior modification

Requirements

  • Completion of a Master's degree in Social Work
  • State licensure as an LCSW
  • Excellent planning, organization, writing, and time management skills
  • Strong ability to communicate with families, professionals, and co-workers
