• Counsels and evaluates clients
• Develops Individualized Family Service Plans (IFSP)
• Maintains progress and monitors outcomes
• Provides Early Intervention (EI) supports to families

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

• Complies with federal and state regulations
• Administers evaluations and assessments
• Develops rapport with children, families, and team members
• Develops joint plans with families and recommends strategies
• Implements IFSP for enrolled families and recommends service frequency
• Provides families with information, skills, and support
• Utilizes coaching model and natural environment teaching
• Attends Birth to Three sponsored trainings, meetings, and learning opportunities
• Completes Birth to Three Service Coordinator modules and functions as the Service Coordinator as needed
• Ensures health and safety of staff and individuals served

• Knowledge of Connecticut’s Birth to Three early intervention program
• Ability to adapt to various learning styles

QUALIFICATIONS

• Master’s Degree or Doctorate of Social Work
• Licensed by Department of Public Health
• Licensed Clinical Social Worker (LCSW) license required
• Valid Connecticut driver's license
• Confidence in adapting to various learning styles
• One year of experience working with children in early intervention preferred
• Demonstrated commitment to Natural Learning Environment Practices
