Responsibilities

• Evaluate patient needs

• Collaborate with healthcare providers to address significant social factors impacting health
• Prepare social histories
• Assess patient needs
• Provide services per regulations
• Support interdisciplinary planning

Requirements

• At least 2 years of experience
• LCSW credential
• Excellent decision-making and communication skills
• Master's degree from a CSWE-accredited school
