Duties And Responsibilities
- Completes initial screening of patients referred from primary care providers to determine disposition and needs
- Employs a wide range of interventions, such as care coordination/case management, risk screening and behavior change, educational information, referrals, and safety planning
- Offers solutions to road blocks while implementing programs
- Makes follow-ups with clinic staff for high risk patients
- Maintains care plans based on patient responses and provider directions
- Performs patient care responsibilities considering needs specific to the standard of care for patients age
- Knowledgeable of medications and their correct administration based on age of the patient and their clinical condition
- Formulates a teaching plan based on identified patient learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate
- Able to perform treatments and provide services and education to level of licensure
- Advocates on behalf of patients and their families to gain access to services and resources
- Provides provider support in complex cases
- Treats patients and their families with respect and dignity
- Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families
- Functions as a liaison between administration, patients, physicians and other healthcare providers
- Interacts professionally with patient/family and involves patient/family in the formation of the plan of care

- Serves the adult population with behavioral health needs, including but not limited to: outpatient individual or group therapy, psychopharmacology, partial hospital programs and inpatient programs, and self-management tools
- Effectively tracks and documents calls, outreach, case reviews, and outcomes of referrals in EMR system
Qualifications
- High School graduate or equivalent
- Current BLS certification including training on defibrillators and renewal as required
- Equipment/Technology: Business machines operation required and proficient in MS Office and Excel
- Ability to learn complex phone systems and hospital information system
- Must be a great listener and have the ability to communicate expertly and appropriately in English with a diverse group of people
- Must be even-tempered and able to make sound decisions and judgments
- Must have the ability to prioritize multiple and competing customer and staff needs
- Must have excellent interpersonal skills by treating others with politeness and civility
