About the role

Serve as a contributing member to the PACE Interdisciplinary Team (IDT). Provide direct, hands-on clinical care to residents, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility. Support residents' physical and emotional well-being by building meaningful connections and delivering compassionate, person-centered care. Monitor and report changes in residents' condition to the nursing team to ensure timely and appropriate care. Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards.

Requirements

  • Current Licensed Clinical Social Worker licensure in KY.
  • Education: Masters’ Degree from an accredited school of social work required.
  • Strong communication and customer service skills.
  • Ability to follow care plans and resident orders.
  • Long-term care experience (preferred).
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license and have means of transportation.
  • Must clear background check.

Qualifications

Pay

$65,000 - $80,000, Based on experience

Schedule

Monday - Friday, 8:00am - 5:00pm, Occasional evenings and weekends
