About the role
Senior Care Therapy is seeking full-time or part-time Licensed Clinical Social Workers to provide in-person psychotherapy services to the geriatric population across skilled nursing facilities, including sub-acute, long-term care, and assisted livings. As a clinician-owned and operated company, SCT provides supportive counseling through patient-centered services that impact residents' mood, functioning, and quality of life while collaborating with facility staff to improve outcomes for residents and care teams.

Key Responsibilities
- Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility
- Assess, diagnose, and treat emotional and behavioral health conditions including depression, anxiety, adjustment disorders, and cognitive decline
- Develop and implement evidence-based treatment plans tailored to each patient's needs
- Collaborate with facility staff, interdisciplinary teams, and family members to support patient care
- Maintain accurate and timely clinical documentation in accordance with regulatory standards using the clinician-designed EHR
- Monitor patient progress and adjust treatment plans as needed
Requirements
- Active and unrestricted license to practice as a Licensed Clinical Social Worker in the state applying for
- Effective oral and written communication in English
- Basic proficiency with technology, including electronic health records
- Strong organizational and documentation skills with attention to regulatory compliance
- Ability to provide in-person services at assigned facilities, up to 45 minutes
- Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day
- Full vaccination and booster status may be required in some facilities
Preferred Qualifications
- Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting
- History of treating anxiety, depression, and adjustment disorders
- Psych or addictions experience is a plus
- Excellent time management and organizational skills
