Licensed Clinical Social Worker

Role Overview

Sought are licensed clinical social workers to provide in-person psychotherapy to the geriatric population across skilled nursing, sub-acute, and assisted living facilities.

Key Responsibilities

• Conduct individual and group therapy sessions.
• Assess, diagnose, and treat emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans.
• Collaborate with facility staff, families, and interdisciplinary teams.
• Maintain accurate clinical documentation using an electronic health record system.
• Monitor patient progress and adjust treatment as needed.

Qualifications & Skills

• Active, unrestricted LCSW license.
• Effective communication skills in English.
• Proficiency with electronic health records and technology.
• Strong organizational and documentation abilities.
• Ability to provide in-person services within facilities.
• Physical ability to move within the facility.

Preferred Experience & Attributes

• Experience in geriatric, sub-acute, or long-term care settings.
• History of treating depression, anxiety, or adjustment disorders.
• Excellent time management skills.
