Seeking a Licensed Clinical Social Worker (LCSW) to provide in-person psychotherapy services to the geriatric population in skilled nursing and long-term care facilities.

Key Responsibilities

• Deliver clinical care in skilled nursing and subacute settings
• Assess, diagnose, and treat emotional and behavioral health issues
• Develop and adjust personalized treatment plans
• Collaborate with interdisciplinary teams and document clinical progress accurately

Qualifications & Skills

• Active, unrestricted LCSW license in the state of practice
• Effective communication skills in English
• Proficiency with electronic health records (EHR)
• Strong organizational and documentation abilities
• Ability to provide in-person services within a 45-minute radius
