Schedule: Days, Monday - Friday 7:30 AM - 4 PM, PRN

Responsibilities

  • Lead Diagnostic Assessments by interpreting complex psychosocial measures to accurately identify mental, emotional, and behavioral disorders, ensuring every patient receives a clear path to recovery.
  • Create Tailored Treatment Plans based on specialized clinical findings, translating data into actionable strategies that directly improve long-term patient health outcomes.
  • Provide Holistic Psychotherapy ranging from urgent crisis intervention to long-term support, offering patients a consistent and compassionate clinical anchor throughout their healing journey.
  • Collaborate with Multidisciplinary Teams and families to align treatment goals, ensuring a seamless, compliant support network that surrounds the patient at every stage of care.
  • Document Patient Care Journeys with meticulous detail to meet federal and state standards, protecting the integrity of the treatment process and ensuring continuity of high-quality care.

Requirements

  • Licensed Clinical Social Worker credentialed from the Texas Department of State Health Services required.
  • Master's degree in Social Work required.

Qualifications

  • Licensed Clinical Social Worker (LCSW): You must hold a current, valid LCSW license to practice independently at the required clinical level.
  • Specialized Clinical Experience: Proven expertise specifically within Transplant or Dialysis social work is essential to navigate the unique complexities of these patient populations.
