Responsibilities

• Conduct comprehensive assessments
• Deliver counseling and crisis intervention
• Coordinate access to community resources and benefits
• Support patients and caregivers through education, advance care planning, and emotional support
• Ensure accurate clinical documentation

Collaboration

Work closely with physicians, nurses, and care managers to develop patient-centered care plans aimed at reducing avoidable hospitalizations and enhancing quality of life.

Qualifications & Skills

• Licensed in clinical social work
• Strong assessment, counseling, and interdisciplinary team collaboration skills
• Ability to navigate community resources
• Provide compassionate, patient-focused care
