About the role
Providing in-person psychotherapy services to the geriatric population in skilled nursing facilities.
Responsibilities
- Conduct individual and group therapy sessions addressing emotional and behavioral health conditions.
- Assess, diagnose, and develop evidence-based treatment plans tailored to each patient.
- Collaborate with facility staff, interdisciplinary teams, and families to support patient care.
- Maintain accurate clinical documentation using electronic health records (EHR) in compliance with regulations.
- Monitor patient progress and adjust treatment plans accordingly.
Qualifications & Skills
- Active, unrestricted license as an LCSW in the practicing state.
- Effective communication skills in English and proficiency with technology and EHR systems.
- Strong organizational skills and attention to regulatory compliance.
- Ability to provide in-person services within facilities, including mobility around the site.
- Full vaccination and booster may be required in some facilities.
Preferred Experience & Traits
- Experience in geriatric, subacute, or long-term care settings.
- Background treating anxiety, depression, and adjustment disorders.
- Excellent time management and organizational abilities.
