Licensed Case Manager (CM III) (Part Time Days)

About the Role

The Licensed Mental Health Professional (CM III) is responsible for managing the care of individuals in crisis, facilitating admission intakes, treatment planning, discharge planning, and coordinating with an interdisciplinary team to ensure comprehensive care.

Responsibilities

• Facilitate admission intakes, educate individuals about consents and authorizations, and conduct thorough Psychosocial Assessments within specified timeframes.
• Maintain clinic records, including documentation of activities and obtaining necessary signatures.
• Collaborate with the interdisciplinary team to develop and implement person-centered Treatment Plans.
• Gather collateral information and coordinate care with external providers to address service gaps.
• Engage with individuals during the discharge process, ensuring safety and meeting their needs.
• Maintain knowledge of Pennsylvania’s behavioral health system and involuntary legal processes.
• Participate in ongoing education and demonstrate competency through assessments.

Requirements

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or related field.
• Current Pennsylvania licensure as LSW, LAC, LPC, LCSW, or LMFT.
• 1-2 years of experience in a mental health setting.
• BLS certification within seven days of hire.
• Compliance with mandatory vaccination policy and credentialing requirements.
• Successful completion of background checks and medical screenings.

Qualifications

• Preferred: ASAM certification and experience in acute mental health crisis settings.

Pay

• Shift differentials: +$2/hr (Mon-Thu nights), +$3/hr (Fri-Sun nights), +$2/hr (Sat-Sun days).
