Target Pay Range: $54,631 - $64,271; *In addition to the base salary, eligible individuals can earn monthly and annual bonuses for services exceeding impact targets*

Schedule: This position requires three evening nights per week and has the ability for a four-day work week (Monday-Thursday).

About the Role

This position is for case management of an outpatient adult evening SUD group at our St. Cloud Campus.

WHAT IS OUTPATIENT SUBSTANCE USE DISORDER (SUD)?

Our Outpatient Substance Use Disorder Service promotes recovery, well-being, and improved quality of life through integrated substance use and mental health services. Our program offers individualized treatment planning tailored to each client's unique needs and goals, recognizing the broad impact substance use can have on all areas of life. Services Include Individual and group counseling Personalized recovery strategies and relapse preventionSupport in connecting with community resources and servicesA holistic approach that addresses mental health, physical well-being, and social support

Duties And Responsibilities

  • Performs chemical health evaluations for clients
  • Develops individual intervention, treatment, and recovery plans
  • Facilitates or leads group sessions
  • Provides diagnostic and therapeutic care to outpatient SUD clients
  • Provides case management services for outpatient SUD clients
  • Maintains necessary documentation of treatment plans, progress notes, attendance, and discharge summaries in accordance with 245G and Agency Policy

Requirements

  • Bachelor’s degree in chemical health or related human services field or exempt under MN Statutes 148F
  • Licensed as a Drug and Alcohol Counselor according to Minnesota Statutes, chapter 148C
  • Clear DHS background study and sexual exploitations screening
