Licensed Adult Behavioral Health Therapist (LISW / LPCC) – Absolute Care: Columbus, OH

About the Role

This position is a core member of the integrated care team, providing short-term, individualized clinical interventions to members with behavioral health conditions (e.g., substance use, depression, anxiety, schizophrenia, bipolar) that impact their overall health and daily functioning. The role emphasizes trauma-informed care, harm reduction, and motivational interviewing to drive behavioral change and adherence to medical recommendations. As the behavioral health subject matter expert (SME), you will collaborate with an interdisciplinary team, including primary care providers, nurses, care managers, psychiatrists, and community health workers, to assess members comprehensively and develop person-centered treatment plans.

Responsibilities

• Provide individual, group, and post-discharge follow-up behavioral health therapy, including biopsychosocial assessments, individualized treatment plans with SMART goals, timely clinical documentation (e.g., SOAP notes), and progress tracking.
• Deliver crisis intervention and de-escalation for members in crisis (via telephone or in person), addressing suicidality, disruptive behaviors, and clinically appropriate dispositions.
• Serve as the clinical behavioral health SME within the interdisciplinary care team (ICT), offering recommendations, referrals, and follow-up for behavioral health aspects of member care.
• Advocate for and provide evidence-based clinical care, including trauma-informed care, harm reduction, motivational interviewing, and cognitive-behavioral therapies.
• Demonstrate trauma-informed care and motivational interviewing in member interactions and staff trainings.
• Maintain close communication with primary care providers (PCPs) and psychiatrists for all members seen for therapy.
• Complete documentation of patient care services in electronic health records within 3 business days of interaction.
• Develop and support individual treatment plans for members with chronic mental illness, such as schizophrenia, bipolar disorder, depression, and anxiety.
• Promote the use of long-acting injectables, as appropriate, to support treatment plan adherence.
• Schedule appointments, promote attendance (including reminder calls and addressing barriers like transportation), and follow up after missed appointments.
• Collaborate with clinical leadership to develop and maintain group therapy curricula tailored to the member population, covering topics like grief, anger, anxiety, aftercare, and wellness.

Requirements

• Clinically licensed in the state of practice with 3+ years of experience in a behavioral health-related field (e.g., social work, mental health, addiction).
• Licenses must allow for clinical practice and may include: LISW, LPCC.
• 2+ years of experience in a community mental health center, FQHC, inpatient psychiatric facility, or similar setting serving clients with complex behavioral health needs.
• Experience with complex government-sponsored populations (e.g., Medicaid, Medicare) preferred.
• Experience with member engagement, transitions of care, interdisciplinary clinical care, and/or case management preferred.
• Telehealth experience preferred.
• Advanced computer skills with proficiency in Outlook, Word, PowerPoint, and Excel.
• Excellent written and oral communication skills, including the ability to adapt communication to the member’s comprehension level.
• Experience working with multiple electronic health records.
• Strong organizational and time management skills to triage and prioritize patient needs.
• Ability to work in a fast-paced, dynamic environment.
• Active CPR certification.
