Qualifications

• Graduate of an accredited Licensed Practical Nurse (LPN) program
• Current Massachusetts LPN license required
• Current CPR and First Aid certification
• Strong communication, organizational, and interpersonal skills
• Able to work effectively as part of a multidisciplinary team

What You'll Do

• Assisting with health assessments and participant wellness monitoring
• S supporting the development and implementation of individualized care plans
• Administering medications and treatments in accordance with physician orders and established protocols
• Promoting health education, wellness initiatives, and preventative care practices
• Collaborating with Registered Nurses, physicians, specialists, families, and interdisciplinary team members to ensure coordinated care
• H Helping maintain a safe, healthy, and person-centered environment for all participants
