Lead Social Services Coordinator, Long Term Care (LTC)

About the role

We are seeking a dedicated team member to provide holistic support within a skilled nursing facility, focusing on residents' social, emotional, and economic needs.

Responsibilities

• Collaborating with the interdisciplinary team to develop and implement individualized care plans
• Facilitating care conferences and investigating grievances
• Conducting assessments and aligning resources to meet resident needs
• Applying age-appropriate care protocols for adult and geriatric populations
• Demonstrating effective communication and building rapport with residents, families, and healthcare staff
• Utilizing critical thinking for independent judgment and teamwork

Qualifications

• Associate degree in Human Services, Social Work, or related medical field
• Experience in long-term care, social services, or human services
• Meet state certification requirements for Social Services

Preferred Skills and Attributes

• Knowledge of psychosocial and economic management of health-related issues
• Strong verbal and written communication skills
• Ability to work effectively with diverse populations and support growth and development needs
