Primary Care Physician Position Overview We are seeking an experienced and compassionate Primary Care Physician (MD/DO) to provide comprehensive, patient-centered care to adult and Medicare populations in a community-based clinical setting. The physician will serve as a lead provider, focusing on preventive care, chronic disease management, care coordination, and the treatment of higher-acuity patients. This position also provides an opportunity to contribute to quality improvement, risk adjustment, and population health initiatives. Key Responsibilities Provide comprehensive primary care services to adult and Medicare patients.Serve as a lead provider for assigned patients and oversee their ongoing medical care.Conduct Annual Wellness Visits (AWVs) and other preventive health assessments.Identify, evaluate, and accurately document chronic conditions and other diagnoses to support appropriate risk adjustment and quality outcomes.Perform post-hospital discharge follow-up visits to promote continuity of care and help reduce avoidable readmissions.Provide additional clinical care and oversight for higher-acuity and medically complex patients.Collaborate with the organization's Quality Programs leadership to support quality improvement and risk-adjustment initiatives.Serve as a physician resource and clinical champion for providers participating in quality and population health programs.Work closely with care management and multidisciplinary teams to coordinate patient services and improve outcomes.Maintain accurate, timely, and compliant medical records and documentation.Participate in initiatives focused on improving patient outcomes, preventive care, care coordination, and patient satisfaction.Follow current clinical guidelines, Medicare requirements, and applicable healthcare regulations. Qualifications MD or DO degree from an accredited medical school.Board Certified or Board Eligible in Family Medicine, Internal Medicine, or another applicable Primary Care specialty.Active and unrestricted medical license in the state of practice.Current DEA registration, as applicable.Prior experience providing outpatient primary care is preferred.Experience with Medicare patients, Annual Wellness Visits, HCC coding, risk adjustment, or population health is highly preferred.Strong clinical judgment and excellent communication skills.Ability to work effectively with multidisciplinary healthcare teams.Commitment to providing high-quality, compassionate, patient-centered care.
