Lead Physical Therapist (PT) In House (FT or PRN)

About the Role

Evolve Therapy Services is hiring a compassionate Lead Physical Therapist for an in-house Skilled Nursing Facility in Orange, NJ. This role involves planning, organizing, developing, and directing Physical Therapy services to ensure the highest degree of quality resident care in accordance with current federal, state, and local standards and guidelines.

Responsibilities

• Provide physical therapy services to residents following established standards, company procedures, and productivity standards.
• Adhere to company procedures for documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
• Conduct regular screenings of residents to determine the need for intervention or treatment.
• Evaluate residents to gather data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
• Perform all other duties as assigned to effectively discharge the responsibilities of the position.
• Complete compliant and medically necessary documentation in NetHealth EMR systems according to CMS expectations.

Requirements

• Minimum of a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in the state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
