Lead RN Case Manager Responsibilities
- Manage your own active patient caseload, conducting regular home visits and delivering skilled nursing care including comprehensive assessments, medication management, wound care, and symptom management
- Lead and mentor the local RN Case Manager team - serving as their first call for clinical guidance, complex case support, and field-level problem solving
- Develop and update individualized Plans of Care in collaboration with patients, families, and the interdisciplinary team, while also reviewing and providing feedback on care plans across your peer team
- Monitor caseload quality among local RN Case Managers, ensuring documentation, care plans, and patient outcomes consistently meet Cherish Hospice standards
- Provide ongoing education to patients and families on the disease process, comfort measures, and what to expect - and coach fellow case managers in doing the same
- Aid in onboarding and orienting new RN Case Managers, sharing your expertise and helping them hit the ground running
- Career transitions across home, assisted living, long-term care, and hospital settings for your own patients and in support of your team
- Participate in interdisciplinary team (IDG) meetings, contributing clinical insight and facilitating case discussions
- Partner with branch and clinical leadership to communicate team needs, flag concerns early, and drive continuous improvement across the Youngstown market
- Maintain accurate, timely documentation in full compliance with Medicare and regulatory requirements
Required Qualifications
- Active RN License in the state of Ohio
- Valid Driver’s License and reliable transportation
- 2+ years of hospice or home health nursing experience
Preferred Qualifications
- BLS Certification
- Experience as a clinical supervisor, team lead, or charge RN
- Strong knowledge of Medicare hospice regulations and IDT processes
